About The Opportunity
This is an exciting opportunity to be part of the dynamic and energetic multidisciplinary TCP team within SWSLHD. The TCP team provides
short-term quality care to our older population. We strive to enable clients to achieve their goals and optimise their function as a continual
part of their recovery process following their hospital stay.

Working in TCP team will provide a rewarding experience helping our older
population to stay in the community as independently and safely as possible. You will be provided with clinical supervision and support to ensure your professional development needs are appropriately.

What you'll be doing
To provide Social Work services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, quality activities and other relevant departmental and administrative duties.

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

  1. Demonstrated experience in a Community Aged Care setting and ability to provide a range of Social Work interventions
  2. Demonstrated experience prioritising work and managing stressful situations
  3. Proven ability to work independently and collaboratively within a multi-disciplinary team.
  4. Demonstrated high level communication skills (written and verbal) and computer skills.
  5. Evidence of commitment to clinical supervision and professional learning.
  6. Demonstrated understanding of, and commitment to quality processes, and the application of evidenced based practice within social work.
  7. Demonstrated clinical knowledge and skills in psychosocial aspects of heath, illness, injury and recovery.
  8. Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
