Social Worker, Psychologist or Nurse – Aboriginal Remote Communities / Family Services Program

ACM Healthcare is working on behalf of an Aboriginal Health Service, we are searching for an experienced Social Worker, Psychologist or Nurse to delivering essential support to the family services program, including case management, team coordination and stakeholder liaison.
The Social Care Practitioner will be based in Katherine with regular travel to rural communities to build relationships with families and stakeholders, guide staff and support program activities in the communities. The Practitioner will also provide specialist support with case management and participate in case conferences.
About the Role:
Work closely with community members, Aboriginal staff and local stakeholders to develop culturally appropriate strategies to recognise strengths and address challenges which may affect the safety and wellbeing of children.
Provide intensive family support for families in the Katherine West region. Support is to be provided through a strengths-based approach, the principles of self determination and respect for Aboriginal people, communities and culture.
Participate in shared planning, with client consent, to respond to the needs of client families.
Provide clinical guidance and support in case conferences.
Assist families to access support, services and resources as required.
Coordinate and provide input into culturally safe and practical programs and education sessions to build parenting capacity.
Liaise closely with the community, local service providers and key stakeholders in the region to ensure optimal and coordinated outcomes for families.
Support staff to identify when families are achieving independence and participation goals, and work with staff to facilitate exit from the program.
This role involves regular travel to remote and isolated communities. The organisation will pay travel and supply a work vehicle. Travel can include 4WD and light aircraft.
Criteria:
Tertiary qualification in Social Work, Psychology, Nursing or equivalent.
Current NT Working with Children Check (Ochre Card) or the capacity to obtain.
Current COVID-19 immunisation certificate.
Willingness to undertake a Police Check.
Hold (or have the ability to hold) an NT Driver’s License
An understanding of and commitment to Aboriginal cultural values.
An understanding of trauma-informed practice and the diverse challenges affecting families in our region.
Previous experience in social work or community development, including case management of high-risk families, service delivery, community consultation, stakeholder liaison, reporting and evaluation and/ or experience working directly with Aboriginal people and/or in remote Aboriginal communities.
