Job Description
ABOUT THE ROLE
In this position you will build support coordination opportunities for NDIS particpants in the Ballina, Lismore and Casino areas. Support Coordination Consultants work in a way that is reflective, and grounded in our organisational values. Through building relationships, following best practice approaches you will work towards providing the best outcomes for our clients.

WHAT WE ARE LOOKING FOR
You will have a thorough understanding of the NDIS and the ability to coordinate, manage and/or facilitate a range of supports to meet identified needs in accordance with participant plans. You'll be a great communicator, have a willingness to learn new skills and enjoy working as part of a cohesive team of experienced consultants.

Criteria:
- Certificate IV Disability / Mental Health / Community Services and or specialised skills to perform at this level.
- A thorough knowledge and understanding of the NDIS and other relevant legislation.
- Knowledge and experience of using positive behaviour support to work with individuals with challenging behaviours.
- Knowledge of Momentum Collective programs and services and other provider services.
- Attention to detail to analyse client outcomes against the NDIS Price Guide and Terms of Business.
- Experience using Microsoft excel, word and ability to adapt to new systems and processes.
