Support Coordinator and Recovery Coach to join our team.
Responsibilities:
Plan Implementation and Coordination:
- Implement all supports outlined in NDIS participant plans.
- Coordinate services within the community to ensure effective support delivery.
- Maintain operational knowledge of the NDIS and its interface with service delivery in the Disability Sector.
- Communicate effectively with stakeholders, clients, and community members.
- Build strong relationships with clients and their families, as well as community and service providers.
- Link clients and their families to community and service providers.
- Participate in stakeholder meetings to contribute to effective service coordination.
- Keep accurate and up-to-date client records on the MCCS client management system.
- Submit reports in relation to clients and programs as required.
- Develop and extend professional knowledge on NDIS requirements and options.
- Follow ongoing risk assessment procedures.
- Support the implementation of all NDIS participant plan supports within the community.
- Build strong, respectful relationships promoting capacity and resilience during complex challenges.
- Coach individuals to increase recovery skills and personal capacity.
- Work collaboratively with participants, families, carers, and other services to design and adjust recovery plans.
- Support participant engagement with the NDIS, applying, monitoring, and reviewing outcomes.
- Manage participant budgets.
- Working knowledge of the NDIS.
- Significant experience in the mental health field, with lived experience highly recommended.
- Driver's license with reliable transport.
- Security check requirements including police clearance, 100 points ID, NDIS Worker Screening Check, and rights to work in Australia.
- Maintained vehicle with minimum third-party insurance.
- Covid-19 immunization certificate.
- Current First Aid/CPR Certificate, NDIS Worker Orientation Certificate, NDIS Supporting Effective Communication Certificate, and Covid-19 Infection Control Training.
Suitable certification or relevant experience.
Experience:
At least 2 years' experience in a similar environment.
Experience in clinical/health, welfare services, and support systems
Skills:
- Understanding of mental health and physical health issues.
- Strong communication, negotiation, and conflict resolution skills.
- Ability to work unsupervised and within a team environment.
- Experience in building and maintaining effective relationships with support organizations.
- Ability to work with various stakeholders including clients, carers, GPs, and support organizations.
- Willingness and ability to travel as part of the role.
- Current driver's license, reliable vehicle, police clearance, and working with children check
