$34.65 – $36.53 per hour + 11.5% superannuation + salary packaging up to $15,900
5 weeks annual leave plus 2 additional wellness days per year!

The position:
This position forms part of our Individualised Services team that specialise in establishing a positive collaborative relationship with participants and their support network, while ensuring the implementation of their plans and supporting participants to identify and connect with supports in their local communities, build skills, overcome barriers, and achieve their goals.
This role aims to promote independence and confidence in our participants, to enable them to live their best lives.
Duties of the role will include:
Play an active role in supporting NDIS participants to build capacity to coordinate their NDIS plans, negotiate and connect clients to appropriate support and services;
Identify, coordinate, manage and or facilitate a range of supports and support providers to meet identified needs;
Regularly monitor expenditure and support participants to remain informed as to their rate of expenditure and the potential effects and;
Provide expert advice and consultation to NDIS participants and their families on the changing NDIS environment to build capacity to understand and navigate service systems.
About You:
To be considered for this role candidates will need to have the below essential qualifications and experience
A minimum Cert IV in Mental Health or Community Services with demonstrated experience providing Support Coordination
Current Drivers License and reliable vehicle with third party insurance;
You will have proven capacity to build rapport with clients from a diverse range of backgrounds and have experience working with NDIS packages plus;
Well-developed IT skills, with experience using Microsoft products and databases.
The successful candidate will also have, or be willing to obtain as a condition of employment the below listed certificates
Current NDIS Worker Screening Check
NDIS Worker Orientation Module Certificate
First Aid Certificate
Working with Children Check
