About the role:
As a Support Worker (Home Care Packages), you will be responsible for:
- Delivering services for clients at home and in the community as outlined in the Personal Independence Plan including supporting and guiding clients to complete identified tasks, as well as completing agreed tasks on behalf of client.
Assisting the service coordinator to maintain appropriate communication with carers and family members nominated by the client.
Adhering to risk mitigation procedures and bring new risks to the attention of the service coordinator.
Advising the service coordinator of significant changes in the client’s capacity and/or if the client requests a different service mix.
Maintaining accurate comprehensive case notes.
Undertaking administrative duties associated with the role.
We have part time and casual opportunities available. 30 hours per fortnight. Monday - Friday

Your ability to maintain confidentiality and impartiality
Respect and responsiveness to client needs
Well maintained, clean & reliable vehicle with full comprehensive insurance
A willingness to work with non-traditional approaches to service delivery for older people\
It is essential that you:
Are fully vaccinated for CoViD 19
Maintain a NDIS Worker Screening check
