About the role
In line with your commitment to customer care, you will have experience working with older people, a proven ability to communicate effectively and be able to follow and implement individual service plans, healthcare and positive behaviour support plans.
Working in collaboration with the Social Activities Coordinator (Outer East) you will be required to undertake a number of activities including but not limited to:
Delivering and facilitating person centred social and planned activities which are goal orientated, engaging and meaningful (in house, community based and remotely) for Older Adults and those with complex needs that support wellness, reablement and independence
Communicating effectively with participants, carers, relevant community organisations for coordination of participant service delivery
Transporting participants when required for program activities (this may include driving a 12-seater bus)
Promoting and growing the services within the community
Ensuring risk management and all relevant program documentation is completed
Although a typical day might be hard to come across, you will regularly be engaging with the clients in groups on outings exploring the great South East or in our centre as they challenge their minds in a variety of activities or even at a simple gathering sharing stories.
About you
If you have experience working as a Support Worker, qualified with Certificate IV Leisure and lifestyle or aged care experience and enjoy variety in your day, you might be exactly the person we're looking for!
In this position, your useful skills in creating fun and interesting activities that support older adults to engage more consistently with their community and peers, will be helpful. A knowledge of and experience in low care environments will be beneficial. Mostly your bubbly and energetic personality will be your key asset.
