ABOUT THE ROLE:
Summary of the role:
We are a community based Transitional Aged Care Program located at the Mercy Hospital in Albury, a public health service affiliated with NSW Health. We provide slow-stream rehabilitation, nursing and allied health care and practical support to older people after an admission to hospital. The Care Coordinator will be the main contact person for the care recipient and their support people. You will be responsible for helping care recipients set goals, coordinating supports, making referrals, liaising with other health professionals and providing clinical assessments and therapies in accordance with your experience and training. You will join an experienced and compassionate team of nursing and allied health professionals, all committed to helping care recipients reach their individual goals, and be safe and confident living in their own homes.

Located in Central Albury
Full time permanent position (job sharing considered)

ABOUT YOU:
A tertiary qualified Nursing or Allied Health Professional
A current unrestricted drivers licence
Post-graduate qualifications in a relevant field highly regarded
At least three years post graduate experience working in an acute, sub-acute, community, aged care or primary practice role
committed to providing holistic, person-centred and goal-focussed care
