About the Role - Team Leader - Care Management
We have a great opportunity for an experienced, highly motivated Team Leader to work in our Aged Care team in Bella Vista, NSW in a Full-time role. You play a role in a critical staff leadership, client relationship management, operational management and business development function within the organisation. In this role you:
- Provides reporting and real time updates to Area Manager and other team members across the organisation in order to track and improve performance - within agreed time frames and KPIs
- Act as an escalation point for clients and team members
- Partner with our Enabling Functions to support
- Lead a culture of agility and innovation - understand, articulate and drive continuous improvement as well as transformation
- Provides recommendations to Area Manager and the Regional Business Development Manager in relation to business development
- Coach, develop and build high performing and resilient teams / individuals in the context of a highly mobile workforce

Success in this role
You have a minimum of 5 years' experience in a case management role or as a leading case manager within Aged Care. You love helping others, you are confident in delivering leadership to client service staff . You are committed to treating others with respect and maintaining confidentiality. You have
- Evidence of full Covid-19 vaccinations
- A diploma or degree in Health or Allied Health field or minimum of 6 years' relevant experience to equate to a health-related discipline
- Strong communication, advocacy and interpersonal skills
- Demonstrated time management, prioritisation and budget management skills
- Demonstrated experience in the provision of case management
- Current Australian Driver's Licence and access to a reliable vehicle
- Experience in the Microsoft Office suite and exposure and/or ability to acquire skills for in-house client database systems
