About the role

The Elective Surgery Reform Program is a Department of Health initiative to improve access to and outcomes of elective surgery in Victoria.

The Virtual Surgery School (VSS) Navigator will be vital in driving and implementing Virtual Surgery School for patients that are identified as needing planned surgery at Grampians Health, by providing coordination and support to patients in the form of pre-operative optimisation.

The Virtual Surgery School Navigator will launch and embed Virtual Surgery School in a safe, patient centred manner and facilitate ongoing engagement of key stakeholders. The Navigator will run the Virtual Surgery School including triage of patients and referral management, outsourcing of referrals and supporting the delivery of VSS content to patients across Grampians Health. Focusing on optimisation of the evolving model of care via sound evaluation processes.

The Virtual Surgery School Navigator will collaborate with a multi-disciplinary team to identify and drive access for patients pre operatively to optimise their health and wellbeing prior to surgery. The Navigator role will encompass a combination of operational and project management responsibilities to further grow and develop virtual surgery school.

Key Responsibilities

  • Establish a collaborative and collegiate working relationship with key stakeholders.
  • Key measures of success of the program
  • Actively engage in discussions and information sharing with multi-disciplinary teams
  • Commit to supporting the sustainability and spread of improvement initiatives both internally and externally.
  • Focus on continuous improvement, with a mandate to consider and propose how current processes can be improved and promote shared learnings.
  • Work collectively with the elective surgery reform team to achieve the goals of optimising surgical throughput, fast track reassessment of patients, support non-surgical management pathways and support complimentary reform
Skills and experience
  • Client Centred Coordinated Service Delivery.
  • Have the ability to screen, triage and refer on patients who would benefit from 1:1 allied health input
  • Independently manage bookings and referrals for Virtual Surgery School
  • Flexibility to assist other projects in the reform project as needed/directed.
  • Ensure clinical documentation is accurate and completed in a timely manner.
  • Identify, support and engage with stakeholders throughout the GRHSP to support the ongoing development of the Virtual Surgery School.
  • Build and maintain positive relationships with stakeholders to ensure engagement in the program and to facilitate a partnership approach.
  • Support a team approach to multidisciplinary care through effective communication between health professionals

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020 - 2024. The classification is Clinical Support Nurse which is paid at $51.76 per hour.
