What you'll be doing
The purpose of the position is to work with health care service providers to increase access to health treatment and prevention services for Aboriginal people as well as addressing issues relating to the management of chronic disease with an added focus on improving the patient journey for Aboriginal people within their communities.
To do this the position will provide health promotion and education programs appropriate to the needs of the Aboriginal community with the primary focus on the Chronic Disease for Aboriginal People Model of Care; to manage the care of Aboriginal people with or at risk of developing a chronic disease; and working within a multi-disciplinary community health team.
The position will also contribute and support the planning, development, implementation, monitoring and evaluation of strategic initiatives and operational programs that are culturally appropriate and accessible by Aboriginal people. The position will be required to:
- Liaise, provide advice, and advocate on matters related to improving the health of Aboriginal people and work in partnership with government and non-government organisations with the aim of delivering integrated and culturally appropriate programs and services for Aboriginal people.
- Support development and promotion of health services including follow up services for Aboriginal People with a chronic disease, such as those discharged from hospital.
- Assist with development of referral pathways for Aboriginal People into appropriate chronic disease management programs.
- Collaborate with other Aboriginal Health Workers and relevant stakeholders to assist health service development to strengthen services and partnerships.
- Raise awareness in the community and among health professionals and support services to address identified deficiencies (GAPS) relating to Aboriginal health.
- Collate and compile program data, prepare program reports and correspondence for management, as required.
All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Selection Criteria * Must be of Aboriginal and/or Torres Strait Islander descent (NB applicant’s race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW) and have demonstrated knowledge of current Commonwealth, State and Local Aboriginal health priorities and ability to effectively and sensitively liaise and communicate with local Aboriginal and Torres Strait Islander people and communities and agencies.
- Hold a minimum Certificate III in Aboriginal Primary Health Care Work (Community Care) or equivalent health training and experience, also a willingness to undertake further education, as required.
- Demonstrated ability to successfully plan, develop, deliver or lead, evaluate and report on health promotion and education programs within allocated timeframes and resources, as required.
- Demonstrated ability to engage and work autonomously to effectively prioritise workloads under limited supervision within a multidisciplinary team.
- Demonstrated skills in client assessment, support, referral and advocacy in health or related field and a commitment to quality improvement with the ability to develop resources for the delivery of culturally appropriate programs.
- Well-developed verbal communication skills with an ability to work effectively with a range of health professionals/ service providers/agencies to ensure coordination of care and appropriate allied health and social services referrals for Aboriginal people.
- Demonstrated computer skills with an ability to maintain accurate and appropriate records in appropriate information systems and collate data in spreadsheets, diaries/calendars, as well as produce routine electronic written correspondence, project plans or educational materials and program reports.
- Current drivers licence Class C and ability and willingness to travel within the LHD for work purposes.
