Our Aboriginal Health team offers cultural, emotional and spiritual support to patients and families and regularly provides:
  • support and participation in family meetings, goals of care and care planning
  • patient advocacy and referrals to other services
  • support in relation to discharge planning and linking to community services
About The Program
Improvement and development of the Monash Health Aboriginal Health and Engagement program is an organisational priority. We work in partnership with Aboriginal and Torres Strait Islander people to develop and implement strategies, programs, and initiatives to improve the health and wellbeing of patients and employees, close the gap on health inequity and provide culturally safe environments and care. We now have an opportunity to join our team in a newly created role, responsible for overseeing and implementing initiatives aimed at reducing the health disparities experienced by Aboriginal patients who access Monash Health. This is a full time, permanent role.
About the Role
This position requires a strong understanding of Aboriginal health, culture and the social determinants of health, as well as the ability to work collaboratively with healthcare professionals, community organisations and Aboriginal communities. If appointed to this role, you'll operate in accordance with The National Agreement on Closing the Gap (the National Agreement) which has 19 national socio-economic targets across 17 socio-economic outcome areas that have an impact on life outcomes for Aboriginal and Torres Strait Islander people.
Working under the supervision of the Operations Director, you'll provide strategic leadership across Monash Health program areas, including collaboration with the Aboriginal Hospital Liaison Service and Aboriginal Health Services within Monash Health. You'll also work in collaboration with the Chief Aboriginal Health Advisor to support strategies to enhance clinical outcomes that are related to the Closing the Gap policy and to support the delivery of Monash Health’s Reconciliation Action Plan and Cultural Safety Plan and build engagement relating to Aboriginal health and wellbeing with employees, management and stakeholders.
About You
  • You must be an Aboriginal and or Torres Strait Islander person to be eligible for the role.
  • Potential candidates must identity as an Aboriginal or Torres Strait Islander person, demonstrate acceptance by their Aboriginal community and actively participate in Aboriginal cultural activities
  • You are passionate about supporting and advocating for Aboriginal and Torres Strait Islanders and dedicated to improving health outcomes for this priority population group.
  • You lead by example, thrive working in a dynamic, fast paced environment and are truly passionate about delivering a high standard of evidence based care to Aboriginal people
You are also:
  • able to develop, lead and implement evidence-based Aboriginal and Torres Strait Islander health improvement initiatives through gathering and analysing data that supports measurable objectives (Close the Gap targets)
  • able to use data to inform decision-making, identify areas for improvement and advocate for necessary resources and support.
  • dedicated to cultural safe healthcare
  • committed to delivering patient centred care and patient advocacy
  • enthusiastic, compassionate and able to work collaboratively
What you need:
  • proven Community Connections
  • Identity as an Aboriginal or Torres Strait Islander person and you’re accepted by the community and participate in cultural activities
  • demonstrated experience working in health and in particular Aboriginal Health
  • Australian working rights
  • Bachelor’s degree qualification(s) in a Health field (Nursing, Allied Health), Social Sciences, Public Health, Business Management, or other relevant qualification
  • Qualification in project management, process improvement, change management or improvement science
  • Postgraduate qualifications in Healthcare, Management, Leadership or Public Health or progressing towards one of these
