What You Will Be Doing
The Aged Persons Community Mental Health Team (APCMHT) is a specialist multi-disciplinary and integrated service that provides a broad range of interventions, care coordination and support for persons 65 years and older experiencing severe and persistent Mental Health disorders and their carers.
The Aged Community Mental Health Clinician will plan, evaluate and review care, undertaking face to face assessments and contributing to recovery care plans. The role requires frequent liaison with external services and the internal multi-disciplinary team.
This position is open to Registered Nurses with Mental Health experience. It is a Temporary 12 month Part Time position working 4 days a week.
What You Need
We anticipate you will have:• Registered Nurse, Social Worker, OT or Clinical Psychologist with relevant registration RPN ONLY: Post graduate qualification in Mental Health or specialist undergraduate Mental Health program• Proven ability in the management of clients with a major mental illness and in the techniques of assessment and crisis intervention, and recovery frameworks• Experience working in an acute inpatient or Community Mental Health setting• Current Victorian Driver’s License
