Key Responsibilities:
- Assessing and Planning: Work with NDIS participants and their support networks to assess needs and develop individualised, person-centred plans.
- Support Plan Delivery: Implement evidence-based support plans, focusing on positive Behaviour Support frameworks and reducing restrictive practices.
- Goal Setting: Assist participants in developing meaningful goals and practical strategies, both in-home and community settings.
- Advocacy and Role Modelling: Act as a best practice advocate and model within the multidisciplinary team.
- Collaborative Teamwork: Engage in collaborative efforts with a multidisciplinary team to enhance participant outcomes.
