About the Role

As a Registered or Enrolled Nurse in our Home and Community Care Services, you will play a pivotal role in delivering top-notch care through assessment, case management, and ongoing service monitoring and review. Reporting to the Clinical Care Manager, your contributions will be key to enhancing the quality of life for our clients. We have both full-time and part-time positions available to suit your needs.

Key Responsibilities

  • Balance individual client expectations with program resources, ensuring clear and empathetic communication.
  • Devise innovative and creative responses to meet client needs using both formal services and informal networks.
  • Offer high-level support to clients, particularly those experiencing grief or requiring reassurance.
  • Collaborate effectively with team members to achieve collective goals.
  • Assist clients in developing care plans within budget constraints and provide regular financial reporting.
  • Utilize excellent Microsoft Office skills to support your work.

Qualifications and Skills

  • Current APHRA registration as a Registered or Enrolled Nurse.
  • Current unrestricted Driver’s License and access to an insured/registered vehicle.
  • First Aid and CPR certification or willing to obtain.
  • Willingness to obtain an NDIS check.
  • Strong interpersonal and negotiation skills, with a commitment to high-quality service delivery.
  • Knowledge of community services, Work Health and Safety, Duty of Care, and client and staff rights.
