About the Role 

As Care Coordinator for Rehabilitation Support Services (RSS) you are required to support and manage the delivery of clinical Support Worker services to clients’ / participants’, thus meeting the clients’ goals, while maximising the efficiency and profitability of the organisation.

The incumbent of the position will play a pivotal role in ensuring that all clients and participants are provided with the support required for them to live a safe and fulfilling lifestyle.

It is envisaged that the Care Coordinator will assist with maximising the rehabilitation potential of our clients and participants, through a capacity building approach.

This role will work collaboratively with the RSS Manager and roster team as required, with some likely overlap with the NDIS Services Manager. 

The Care Coordinator is expected to present a positive and professional image when interacting with employees, clients’ / participants’, family members, advocates, service providers, funding bodies and the general community. 

Key deliverables of the role include the review and building of community support plans, support worker supervision and mentoring, delivery of support worker training, providing a clinical resource to the wider team, and involvement in the review and onboarding of new clients / participants.

About you

Qualifications, skills and experience. 

  • Work Rights - permit to work in Australia
  • Nursing back ground (Enrolled or Registered Nurse may be considered) 
  • Current Drivers Licence (essential)
  • National Police Check
  • NDIS Worker Screening Check (or willing to obtain)
  • Experience in the disability/aged care/community sector 
  • Strong attention to detail 
  • Ability to manage workload
  • Excellent time management skills
  • Confident MS Office user
  • Exposure to CRM software
  • Excellent communication skills 
  • Positive and friendly personality, who is open, flexible and willing to be part of an innovative and supportive team who work together
  • At least 3 years of relevant nursing experience, and / or experience in a similar Care Coordination role.
  • Experience in the delivery of training.
  • Ability to work in a team & individually
  • Ability to build rapport with a diverse range of customers
  • Experience working with customers with complex support needs
  • Driven, energetic & committed
  • Strong communication skills
  • Positive and friendly personality, who is open, flexible and willing to be part of an innovative and supportive team who work together
  • Mental Health expertise (desirable)
  • Experience in Acquired Brain Injury or Spinal Injury (desirable)
  • Experience with a wide variety of physical and acquired disabilities (desirable)

This role is expected to commence in July 2024 (by negotiation).
