Your work will involve Home Care Packages, Commonwealth Home Support Program, Transition to Care Program, fee for service work and any new programs that emerge from the Aged Care reform process.As the Care Coordinator, you will also provide effective supervision and support to designated staff; ensure administration, data and financial records are current and support group activities, consumer forums and continuous improvement efforts.To be successful you will have:Qualifications within Allied Health, Enrolled Nurse (RN2) and/or Diploma in Community Services orDemonstrated experience (minimum 3 years) experience in the Aged Care Sector.Relevant experience in care coordination, care management and assessment and a demonstrated ability to provide delivery of services to the aged within their homes.Experience supervising small teams with a high level of motivation and a team orientated approach.Prior experience or, at a minimum, demonstrated commitment to working respectfully with Indigenous and culturally and linguistically diverse clients, communities, staff, and Aboriginal controlled organisations.For further information on this exciting opportunity, please contact Jenny Lintern, on (08) 8951 8000 or visit our careers page www.anglicare-nt.org.au/careers/To request a full position description, please email our HR Team at hr@anglicare-nt.org.auApplications close COB Friday 23 February 2024Please note: It is a requirement that prior to commencing in this role, you must possess a Working with Children (Ochre Card), a National Criminal History Check (within 3 months) and a current Northern Territory driver’s license.As part of the recruitment process, you will be required to address the key selection criteria on the online application on the Anglicare NT website.Experience working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.Anglicare NT is a Child Safe, Equal Employment Opportunity (EEO) employer and strongly encourage Aboriginal and Torres Strait Islander people to apply.COVID-19 Vaccination StatusReceiving and maintaining vaccines can reduce health risks and is one control mechanism to maintain the health and wellbeing of all employees, contractors, volunteers, visitors and the people we work with.
