About the role

This role is part time .8FTE. The purpose of the position is to provide specialist and intensive assistance to help clients understand and access Aged Care services and connect with relevant supports in the community. The Care Finder target population is people who are eligible for Aged Care services and have one or more reasons for requiring intensive support to interact with My Aged Care and/or access other relevant supports in the community. 

Care finders will provide face to face support to people from the target population to help them identify the best options for care to meet their individual needs and goals, to exercise informed choice and to understand their entitlements. 

Your proactive approach and positive attitude will ensure our clients receive the best possible service in a manner that is reflective of STAR’s community Services’ values of respect, dignity and empowerment.

What you will be doing but not limited to

  • assertive outreach to identify people who fall within the target population for Care Finder 
  • engagement and rapport building with potential clients and local intermediaries
  • supporting people to interact with My Aged Care so they can be screened for eligibility for Aged Care services and referred for assessment 
  • support to explain and guide people through the assessment process including, where appropriate, attending the assessment 
  • support to help people to find the Aged Care supports and services they need and connect with other relevant supports in the community
  • high level check-in with clients on a periodic basis and follow up support once services have commenced

Essential Criteria

  • Tertiary qualifications in a related field (e.g. social work, human services, Aged Care, community services or health) and/or relevant experience and/or commensurate experience
  • Attained previous experience in a Community Services industry (preferably Aged Care), service or an equivalent level of expertise and experience to undertake the range of activities required.
  • A commitment to work within the Aged Care policy and fundingframework
  • National Federal Police Check
  • NDIS Workers Screening Clearance
  • Current First Aid and CPR
