MCWA is a in-home aged care provider with 40 years experience that focuses on supporting the multicultural ageing and elderly communities in Australia. As we are growing continuously, we are now seeking an experienced Case Manager to join our Home Care Packages team at our new head office in Bundoora!
About the role
Exciting opportunity for a person who is passionate about aged care, and supporting the ageing and elderly. The successful candidate will have a strong client service focus with demonstrated ability to maintain strong ethics and care.
To be consider for the role you are self motivated and are passionate about making a positive difference for the ageing and elderly and you understand that the role requires a combination of both reactive and proactive measures. You thrive in a fast pace environment and are a great problem solver.
Some of the responsibilities for this position include:
- Demonstrated ability to build strong partnership relationships with client
- Provide comprehensive case management for each client
- Build strong Care Plans that are appropriate for the individual needs of each client whilst meeting the limitations of each client’s budget
- Re-assess client care plans, goals, OHS assessment, and emergency plans for clients
- Maintain strong client notes
- Strong problem solving skills
- Nimble in their ability to support clients
- Recognise, anticipate, and focus on specific client needs and advocate where required
- Work with the Finance team to accurately submit client claims to Services Australia
- Approve monthly client statements prior to mail out
- Establish, coordinate and manage third party services for clients
About You
- Ability to speak Macedonian, Greek or Italian is preferred but not necessary
- Registered Nurse looking for a change
- Proficiency in computer skills
- Excellent communication and interpersonal skills
- Ability to work independently and as a part of a multidisciplinary team
- Valid driver's license
