MACS and Diversitat have been serving the Geelong community for more than 40 years with a focus on our multicultural communities. As the new entity, Cultura operates across various sites delivering a range of services including residential aged care, home care and aged support services, settlement, training and education, financial counselling and disability support. We also operate a range of social enterprises including The Pulse community radio and various community events including the popular Pako Festa.
Cultura also offers an extremely generous salary packaging benefit, giving staff the opportunity to salary package up to $18,550 per annum of their pre-tax yearly income.
The Role:
The Family Services Program (FSP) Case Manager will work with children, young people and their families experiencing complex issues by implementing, monitoring and reviewing client outcomes against the agreed goals of each Family Action Plan. Client engagement with the FSP Case Manager is on a voluntary basis and clients will be determine intervention goals with the assistance of the FSP Case Manager.
This position will be contracted on a maximum-term full-time basis for 3 months.
About You:
- Engage with allocated FSP clients to conduct a holistic needs assessment and develop comprehensive and achievable Family Action Plans;
- Provide case management to clients, implementing activities detailed in the Family Action Plans;
- Keep timely and accurate client records including relevant data as per contract requirements;
- Ensure all required reporting is completed within the required time frame;
- Build and maintain good working relationships with key stakeholders;
- Participate in broader team and Divisional activities;
- Implement, monitor and review client outcomes against Family Action Plans;
- Actively manage individual case load including engagement with new clients, review and exit existing clients;
- Facilitate an integrated care team case management approach;
- Liaise, consult and advocate with external services and other family members as appropriate;
- Meet KPIs as specified in Annual Performance Discussions and Review Plans;
- Tertiary qualification in Social Work (Bachelor of Social Work or Bachelor of Psychological Services), or similar field.
