About the role
The Case Manager is an integral part of the Home Care team. Responsibilities will include but are not limited to -
- Working closely with the client to identify care needs, goals and preferences
- Overseeing the implementation of care and services as agreed with the client to meet their assessed needs
- Managing client Home Care accounts, ensuring appropriate use of funds to achieve desired outcomes
- Monitoring & responding to the changing needs of the client
- Conducting in-home assessments & reviews
- Collaborating & communicating with clients, families, internal & external stakeholders
Skills and experience
- Previous experience in Aged Care services and an understanding of the aged care industry, including Home Care Packages and the Aged Care Quality Standards
- Tertiary qualifications in nursing, allied health, social work, care management and/or related experience
- Strong organisational skills, with the ability to prioritise multiple tasks, meet deadlines & reporting requirements
- Sound IT skills
- Be self-motivated and able to work autonomously
- Strong attention to detail
- Ability to maintain confidentiality
