Job Description: As a Centre Manager – Regional at GNA Services, you will play a crucial role in overseeing the delivery of services to individuals receiving NDIS Services, IHP Services with Southwest WA Country Health Services, DVA Services and Staffing Services along with stakeholders and clients. Your primary responsibilities will include service planning, coordination, and collaboration with internal and external stakeholders to accomplish the objectives specified in participants client plans and service agreements.
Key Responsibilities:
-Strengthen and enhance participants' ability to coordinate GNA services, manage and direct supports, and provide assistance in resolving problems or issues that arise, in accordance with their responsibilities under service agreements, including when changing or ending a service agreement.
-Provide guidance and support to project managers and service managers, coordinators to ensure the effective delivery of services.
-Liaise with various stakeholders including GNA services, programs, partners, funding bodies, and government departments.
-Create and design a service plan for those with complex and specialist needs including where collaboration with, and accountability to multiple services is required.
-Liaising and consulting with other services, Contribute to program evaluation, reporting, and compliance with audit requirements. Provide advisory support to other departments on community engagement strategies.
-Address complex barriers coordinate responsive, individualized support to participants with complex needs, behaviours of concern, medical needs, or other high-risk circumstances of GNA Services
-Assist in organizing events and promotional activities related to the Health and Welfare Services of GNA Services
-Maintain accurate data records and submit reports as necessary.
-Promote the GNA services providers and potential participants and service users.
-Maintain data collection systems as required by funding authorities and submit data and information reports as required.
-Working with Department Child Protection, families, and children to enhance GNA health and welfare services for their capability and smooth transition to social-economic environment for school-leavers with mental and Physical challenges.
-Working with the Department of Justice for Social Inclusion and transition from Jail/Custody to the community.
-Provide reports on specific goals, outcomes, and success indicators within the agreed reporting frequency.
-Effective networking internally and externally to assist cooperate team to achieve their desired plan outcomes.
-Drive results through proactive planning and execution.
-Lead and manage portfolios of projects and cross-functional teams.
-Oversee program delivery, including budget management and evaluation.
-Develop and implement innovative community engagement programs.
Mandatory
-Education: Bachelor/master’s degree in Community Service, Social Work, Sociology, Psychology, Behavioural Science, or related field.
-Experience: Minimum of 3-year experience in a relevant role in a community / social welfare / disability organisation.
