We have a great opportunity to job share, part time as a Chronic Disease Nurse working with the local Aboriginal and Torres Strait Islander community.
We are now seeking a Registered Nurse who is passionate about quality care delivery and education surrounding Chronic Disease management.
The role of the Chronic Disease Nurse is to work with clients that have been identified as having a Chronic Disease; including working closely with AWAHS GPs, Practice Nurses and Aboriginal Health Practitioners around the preparation of Chronic Disease management plans and health checks, involvement in disease specific clinics, referrals to allied health services, referrals to health programs, and regular review of recall register.
If you have an interest in health promotion activities designed to promote community wellbeing through health information, promotional brochures, community development & self-care initiatives, then this is the job for you.
You will ideally have experience working in Chronic Disease management, developing and managing Chronic Disease care plans and running effective Chronic Disease clinics.
