The CHSP Coordinator Role
The CHSP Coordinator aims to provide a comprehensive Community Care Program including identification of needs, assessment, implement, coordination, maintenance and evaluation and review of home and community-based programs within the parameters and criteria of the Eyre & Far North Local Health Network (EFNLHN)
As a member of a multi-disciplinary team, the Community Care Coordinator assists in the development, implementation, coordination and evaluation of community-based ageing and disability programs to support those who wish to remain living in their own home with some assistance.
This is a part time vacancy, working 52.50 hours per fortnight and salary will be adjusted according to actual hours worked.

Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.
