We are currently hiring for the position of CHSP - Program and Wellbeing Coordinator. (CHSP)

Lutheran Homes Barossa is currently seeking a motivated and experienced Group Social Support Coordinator to lead the planning and delivery of programs that maintain and enhance the physical, emotional, and social well-being of older persons, frail aged based on their individual needs and interests.

Your strong organisational and problem-solving skills will enable you to optimise systems, processes, and practises to ensure the best outcomes for our clients.
The Role:
- Relevant qualifications and/ or experience in the Community Services sector including a sound knowledge of the Commonwealth Home Support Program (CHSP)
- Ability to develop and maintain effective relationships with a range of stakeholders.
- Supporting clients and their families to exercise informed choice about the care and services they receive.
- Maintaining and updating in a timely manner all client case notes and client files.
- Identifying, initiating, and coordinating quality improvement activities in relation to client care including quality audits to ensure compliance.
- The ability to create, implement and report on successful social support programs for identified members of the community.
- A sound understanding of the Aged Care Quality Standards
Our Ideal Candidate will have:
- Previous Aged/Home Care experience desirable
- Advanced analytical, organisational and time management skills.
- Valid Police Clearance less than 6 months old (or provide a receipt at interview stage).
- Exceptional customer service skills and interpersonal communication skills.
- Proficiency in using the Microsoft suite.
- A current Driver's License.
- Full vaccination for COVID-19 (minimum 3 vaccinations) and influenza.
