About The Role

The purpose of this role is to provide clients with consistent, holistic support and increase client satisfaction through regular interactions. Offer complete support, understand the evolving clients' needs and codesign services to meet client requirements. Act as the primary point of contact and direct client requests to the appropriate department.

The Client Relationship Coordinator is responsible for undertaking client re-assessments and regular reviews and developing, implementing, and evaluating support plans, ensuring the service is responsive to the needs of clients to optimise their independence and wellbeing.

  • Offering Seamless Experience: Ensuring the current clients receive a seamless experience within the organisation by accessing one or multiple services.
  • Understanding Specific Needs: Developing a deep understanding of the unique needs and preferences of clients, including health, mobility, and social requirements.
  • Personalised Communication: Maintaining regular, empathetic communication with clients and their families to build trust and understand their concerns and expectations.
  • Issue Resolution: Addressing and resolving issues and concerns about care quality, staff interactions, facility amenities or other matters of care and service.
  • Care Plan Oversight: Collaborating with specialists internally and externally to ensure client care plans are up-to-date, comprehensive, and effectively implemented.
  • Feedback Implementation: Gathering and acting upon feedback from clients and their families to enhance care services and facilities.
  • Client Advocacy: Acting as an advocate for clients, ensuring their rights and interests are represented and respected.
  • Family/Carer Support: Providing support and information to families and carers about aged care processes, options, and decision-making.
  • Awareness: Understanding trends in aged care, including regulatory changes, to anticipate and meet changing client needs.
  • Interdepartmental Collaboration: Working with internal teams cohesively and in an integrated approach to support client care.
  • Fees Management: Understand all options of funding within Aged Care, Disability, Medicare, Private Health Insurance or Private Funding to offer solutions that meet the client's needs

About You

  • Tertiary qualifications in Health, Social Work, Community or similar (Desirable)
  • Demonstrated experience in Aged Care, Disability or Community Care sector
  • Demonstrated ability to create and maintain Client Relationships in similar roles.
  • Ability to fluently speak a second language (Desirable)
  • Demonstrated knowledge of quality standards within Aged Care, NDIS or Allied Health services.
  • Ability to communicate clearly and effectively.
  • Ability to plan, implement and evaluate services that meet the needs and interests of individual clients.
  • Ability to positively interact and collaborate with all stakeholders.
  • Critical thinking skills, including the ability to analyse and challenge conventional practice and ideas.

The successful candidate will require National Police clearance prior to appointment (completed by 3Bridges), valid NSW Working With Children Check clearance and Current NSW drivers’ licence.
