About the role:
As the Clinical Care Coordinator, you will lead both clinical and care teams to ensure comprehensive resident care. Aligned with the facility's mission and values, the coordinator focuses on holistic care while upholding the legacy of the organization.
The successful individual will:
- Coordinate care, ensuring resident safety, quality outcomes and an unparalleled resident experience
- Provide clinical leadership, ensuring accountability and excellence in the delivery of care
- Develop individual and system-wide improvement plans
- Build strong relationships with families and residents
- Ensure compliance with privacy principles and uphold the confidentiality of residents
- Lead quality improvement initiatives in collaboration with the management team
- Uphold the aged care values and actively participate in fostering a supportive workplace culture
- Registered Nurse with AHPRA registration
- Minimum of 3 years’ management experience in aged care
- Commitment to integrity, honesty, and trustworthiness
- Strong communication, negotiation, and IT skills
- Current National police check and knowledge of WH&S and Infection Control
