Clinical Care Coordinator - Aboriginal Health Unit

An exciting and innovative opportunity is available for an appropriately skilled nurse to join Latrobe Regional Health and its Aboriginal Health Unit.

In this position you will coordinate integrated care for patients of Aboriginal and/or Torres Strait Islander origin within our services and facilitate care within the community. You will  be integral in increasing patient and carer’s knowledge, confidence, and participation in their self-management, treatment and care planning, and will love the difference you can make every day you come to work.

You may already have experience in a similar role or could be looking for the next step in your career – either way you will be well supported through our multi-disciplinary teams and a range of staff well-being activities.

 Our ideal candidate will:

  • Be a Registered or Enrolled Nurse with AHPRA
  • Have sound knowledge and understanding of Aboriginal culture, society and kinship networks as well as the ability to communicate with, and be accepted by, the Aboriginal community. 
  • Have previous experience with coordinating care for patients in the community
  • Be of Aboriginal or Torres Strait Islander descent (please note: this is NOT mandatory).
  • Well-developed interpersonal and communication skills, written and verbal, and the ability to develop and maintain communications with Aboriginal communities, staff groups and otehr key stakeholders.
