Our client, a distinguished not-for-profit (NFP) aged care provider, is actively seeking an experienced Care Manager to join their team immediately at their state-of-the-art facility comprising 121 beds. This organization is deeply committed and supportive, emphasizing a culture of trust and respect for the elderly generation, thereby ensuring a rich and fulfilling life experience.
Responsibilities:
- Conduct comprehensive client assessments and develop individualized care plans.
- Coordinate access to healthcare and social services, advocating for clients' needs.
- Serve as the primary point of contact and advocate for clients and their families.
- Maintain accurate client records and participate in quality improvement activities.
- Contribute to the development and implementation of new care initiatives and protocols.
Skills and Experience
- Sound Knowledge of ANACC, care planning, Aged Care standards and auditing
- Experience working as a Care Manager within aged care or as a senior Nurse.
- 4 years minimum as RN in aged care sector
- Leadership experience is essential.

- Have full working rights in Australia.
- APHRA Registration
