About the role
The purpose of this role is to provide advanced clinical expertise and care relevant to the person who requires rapid assessment, navigation and connection to community based health care services provided either by Metro North Health or non-government healthcare related organisations. The position will lead the further development of clinical practices within the Rapid Access to Community Care (RACC) service through leadership, role modelling and application of the principles of contemporary evidence based consumer/person focused care, and education.
