Monash Children’s Hospital continues our tradition of providing outstanding patient and family centred care in a purpose-built paediatric facility, delivering clinical services in a child and family-friendly environment. The Stepping Stones Adolescent Inpatient Unit is a 15 inpatient beds and 5 transition (day) beds Ward. It is a state of the art, purpose-built facility housed in the new Monash Children’s Hospital. The Unit offers Mental Health interventions to adolescents (aged 12 to 18) who present with acute and severe mental health problems, associated high risk factors and have difficulty maintaining their own safety and the safety of others.
About ELMHS
Early in Life Mental Health Service (ELMHS) is a place where young people who are experiencing emotional, behavioural or mental health problems can get help. ELMHS provides a range of mental health services to pregnant mothers, infants, children and adolescents and their families within the Southern Region.
About You
The Collaborative Transitional Care Worker is an experienced Mental Health Clinician with previous experience within a youth / adolescent mental health setting. You have exceptional communication and negotiation skills, are able to work & relate well with young people, related services and their families within the multidisciplinary team setting. You are also able to work both independently and at the same time work collaboratively with the various components of the ELMHS service. The role provides an opportunity to develop your capabilities of working within a diverse Early in Life Mental Health Setting; and extending your knowledge and skills at liaising with external agencies. We are looking for someone with:
  • Qualifications and relevant registrations in at least one of the following: Nursing (Div. 1), , Social Work, Occupational Therapy, Speech Pathology.
  • Capacity to work independently and lead a team
  • A friendly and professional attitude
  • Excellent engagement, relationship and therapeutic skills.
This is a Mental Health Clinician position as follows:
  • Nurse – RPN 4 Year 1-3
  • Social Worker – Grade 3 Year 1-4
  • Speech Pathologist – Grade 3 Year 1-4
  • OT – Grade 3 Year 1-4
A Day in The Life
An exciting and rare opportunity exists for a suitably qualified and experienced Clinician to work within the Early in Life Mental Health Services. The Collaborative Transitional Care Worker role for the Adolescent Unit is that of a full-time mental health clinician of any discipline (Occupational Therapy, Social Work, Speech Pathology and Nursing). The position does not operate as a bed manager, rather it is an integral part of the clinical processes for the unit and involves managing admission and discharges with referrers to ensure effective patient flow management and high-quality care. In addition to this, the key components of the role include:
  • Receiving referrals for planned and unplanned admissions to the Adolescent Unit.
  • Liaising with the referrer regarding appropriateness of the referral.
  • Co-ordinate all preadmission meetings & liaising and providing consultation with internal and external service providers regarding admission and other treatment options.
  • Working with the ANM /Nurse in Charge to assist in decision making in relation to Low Dependency (LDU)or Intensive Care Area (ICA) bed allocation
  • Repatriation of Out of Area young people admitted to Stepping Stones to their appropriate unit.
  • Conducting pre-consultation meetings with the referrer, the young person and relevant supports to ascertain treatment goals.
  • Attend Care team meetings to support Community teams managing complex young people as part of comprehensive treatment planning as required.
  • Establish and maintain close relationships with Paediatric Emergency Psychiatric Service and Paediatric Consultation Liaison
  • Work closely with ELMHS community based services, YPARC and Unit 2 where required, building up a shared understanding of pathways into these services.
