Job description

Come and join Australia's most innovative and dynamic home health organisation!

Hi there, we're Five Good Friends, an approved aged care and registered NDIS service provider. We help our clients live vibrantly in their own homes by wrapping a care team around them and their families.

Our vision is to keep people connected to and engaged with the friends, family, homes and communities they love for as long as they desire.  To do this well we need to provide great support along every step of their journey.  Today we are looking for the right people to support some of their first steps.   If you share our vision and commitment to people we welcome you to apply to be a full-time Community Onboarding consultant for Five Good Friends in Brisbane and greater regions.

About the Role

You'll be working as a Community onboarding Consultant in our Welcome Team.

You’ll support business growth by signing up new members, ensuring they and their families understand all of Five Good Friends products and services, pricing and fees and overall value proposition. You’ll also help them navigate and understand the relevant government funding systems, whether on a Home Care Package, NDIS or as a private customer.

You’ll complete a detailed assessment and capture all necessary information about new member needs and goals. This is the beginning of the onboarding journey with Five Good Friends and is critical for the development of great care plans and sourcing the perfect support person for the member and their families.

You’ll be an advocate for the use of our propriety software, inviting members and their families to use the app to their maximum ability.

This role plays an incredibly important part, in setting up new members and their families for success.  Ensuring a smooth and happy transition along the onboarding journey to a Care Planner and ultimately into long-term care and support management with Five Good Friends.

If you believe support for our loved ones needs to change, then please join us in creating a better, brighter, future by applying to be a Community Onboarding Consultant within our Welcome Team.
