- Assesses community needs and resources for health, welfare, housing, employment, training and other facilities and services.
- Liaises with community groups, welfare agencies, government bodies and private businesses about community issues and promotes awareness of community resources and services.
- Working with clients from diverse backgrounds to ensure their individual needs are supported while respecting their culture.
- Delivering services with compassion, integrity, and dignity.
- Assist clients in accessing the community.
- Assist clients in achieving rehabilitation goals.
- Support clients with judgment and decision making.
- Monitor client behaviour.
- Maintain accurate records and report writing.
- Provide feedback to the relevant Manager.
SKILLS & EXPERIENCE:This position will require the following
- Relevant qualification in community services and at least 2 years work experience
