An exciting opportunity exists for a Team Leader to join the Day Rehabilitation Program at Bulli Hospital
Where you’ll be working
is a 120 bed facility offering sub-acute Aged Care, Rehabilitation and Palliative Care services across four wards. Bulli works closely with the Aged Care, Rehabilitation and Palliative Care Specialist team across the Illawarra Shoalhaven Local Health District to ensure a safe and timely transfer of patients who have completed an acute episode of care and require ongoing care and management. Bulli Hospital also provides outpatient Geriatric Services and a Day Rehabilitation Program. The outpatient services are supported by senior medical specialists, specialist nursing staff and Allied Health. The outpatient unit houses therapy and assessment areas for patient appointments. In addition, Bulli Hospital provides treatment for minor injury and illness, similar to a GP, via the Urgent Care Centre. This model of care enables patients to be seen and assessed by specialist nurse practitioners or medical officers to deliver treatment and management plans.
What you'll be doing
The Day Rehabilitation Program (DRP) Team Leader is the leader of an interdisciplinary team that provides a coordinated rehabilitation service. The DRP provides care to clients who would benefit from an interdisciplinary coordinated Rehabilitation Program. The DRP Team Leader is responsible for all aspects of clinical service provision, including referral management, direct patient care, co-ordination of the program, clinical activity, and outcomes. The DRP Team Leader is also responsible for operational management of the service, including aspects such as work health and safety management and leave coordination.

Selection Criteria: * Any profession that holds a current relevant tertiary qualification under the NSW Health Professional (State) Award (Physiotherapy, Occupational Therapy, Speech Pathology, Dietitian, Social Work, Podiatry, Counsellor, Podiatrist) and corresponding registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional association. Psychology qualifications will be deemed equivalent qualification as a Counsellor under the NSW Health Services Health Professionals Award.

  • Extensive rehabilitation clinical experience with sound knowledge of rehabilitation principles and interdisciplinary care with extensive post graduate experience in any of the above mentioned Allied Health disciplines.
  • Demonstrated skills and knowledge in case management, health coaching and/or enablement techniques for people with a variety of health conditions (such as stroke and deconditioning) using advanced clinical skills in assessment and care planning.
  • Demonstrated ability to lead a team and collaborate with others to achieve positive outcomes.
  • Extensive experience working in a multi/interdisciplinary team across a variety of service delivery models.
  • Demonstrated ability to access, utilise and analyse clinical data to evaluate and direct the performance of a clinical service.
  • Demonstrated high level written, computer and verbal communication skills, especially negotiation and collaboration.
  • Current NSW drivers licence and willingness to travel to various sites as required.
