AddressThe Diabetes Connect program provides support of local integrated pathways for people living with Type 2 diabetes as part of the Diabetes Connect pilot. People with type 2 diabetes are at high risk of poor health outcomes and this can be exacerbated by existing barriers that are potentially avoided.
The aim of the Diabetes Connect program is to target the settings of those with the greatest risk of poor health outcomes and greatest economic and social need, and who may face barriers to accessing care through other services/settings.
The Diabetes Connect Promotion Officer provides essential consultation for and promotion of the Diabetes Connect program with health services, social services, and those with lived experience of Type 2 Diabetes.
