United Support Services is a professional and unique organisation specialising in supporting a group of people with a disability and high care requirements for a peaceful and meaningful life. We aim to allow people with disabilities / special needs between 0-65 years of age to live a more ordinary life and achieve personal goals while receiving quality and flexible care entirely funded by the government.We are looking for a dedicated Disabilities Services Officer to work closely with our residential care officers and care/support workers to provide education and community access to people with intellectual, physical, social and emotional disabilities.Roles and responsibilities include:
- Conducting interviews with clients and their families to assess the extent and nature of their difficulties.
- Evaluating clients' needs and creating, developing, and implementing educational, training, and support programs.
- Helping clients with addressing social, emotional, and financial issues.
- Monitor and report on the progress of clients, maintain client records and monitor the outcomes.
- Referring clients to additional support agencies and allied health agencies as required.
- Assist with applying for and monitoring a person's individual funding to ensure it meets person's support needs.
- Mentor support and care staff and our support coordination team to provide ongoing training and development to deliver a high standard care.
- Working with our support worker and our support coordination team to provide education and community access to our clients with disabilities.
- Minimum qualification of AQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2) or at least 3 years of relevant experience in a related field
- 2 years minimum experience as a Disabilities Services Officer or in a related field.
- Excellent communication and interpersonal skills, including demonstrated experience in interacting with a wide range of people.
