Key Responsibilities:
Participant Intake and Assessment:
- Conduct initial assessments to determine eligibility for NDIS and other disability support services.
- Collect and review necessary documentation from participants.
- Facilitate the completion of application forms and related paperwork.
Case Management:
- Create and maintain detailed participant records in the case management system.
- Coordinate with other NDIS and disability support staff, healthcare providers, and community organizations to facilitate participant support.
- Monitor participant progress and provide updates to relevant stakeholders.
Communication and Support:
- Provide information and guidance to participants and their families about organisational processes.
- Ensure participants understand their rights and the services available to them.
- Respond to participant inquiries and resolve any issues that arise during the intake process.
Compliance and Reporting:
- Ensure all processes comply with statutory guidelines and regulations.
- Prepare and submit regular reports on intake activities and participant progress.
- Maintain confidentiality and handle sensitive information with discretion.
Training and Supervision:
- Train new staff on procedures, policies and care assistance services.
- Supervise intake staff, manage their performance, and provide ongoing support and development.
- Conduct regular team meetings to discuss updates, share knowledge, and address any issues.
Leadership and Team Management:
- Ability to build and maintain a motivated, cohesive team.
- Skill in addressing and resolving conflicts within the team or with clients.
- Providing ongoing training and professional development opportunities for staff.
Performance Monitoring and Evaluation:
- Implementing quality assurance measures to monitor and improve service delivery.
- Establishing feedback mechanisms to gather input from clients and staff.
- Tracking and evaluating outcomes to ensure the effectiveness of the intake process.
