About the Organisation

Derby Aboriginal Health Service (DAHS) is an Aboriginal Community Controlled Health Service (ACCHS) providing holistic primary health care services to the Aboriginal and Torres Strait Islander people living in Derby and surrounding communities. DAHS provides a wide range of innovative public health programs in addition to direct comprehensive primary health care services. It is required to comply with a number of Service Agreements with Commonwealth and State departments and agencies, and to operate with a number of government and private medical and health delivery organizations.

About the Opportunity

DAHS has a rare and rewarding opportunity for a Disability Services Case Coordinator to join their multidisciplinary team in Derby, WA. This role is being offered on a permanent, full-time basis.

The Disability Services Case Coordinator is responsible to provides assistance to NDIS participants to understand what is included in the NDIS plan, and supports participants to identify goals and needed supports and to choose and connect to suitable Disability Support Providers. The Case Coordinator can also assist the participant to strengthen their abilities to advocate for and coordinate supports & participate in the community. The role requires knowledge and understanding of the disability sector and service standards, human rights and the principals of person-centered plans and the participant’s individual choice and control regarding supports. Creating and maintaining strong professional networks within Disability Service Providers is essential in order to assist participants with access to appropriate supports that meet their needs. All applications are to be in writing and must address the selection criteria.

DAHS provides primary health care to Derby and surrounding communities. DAHS is also a part of a Kimberley network of Aboriginal Health Services.

More specifically, your day-to-day duties will be in the following key areas:

Understands person centered approach and enables participants to implement plans in a customised and practical way.
Understands and promotes the principles of choice and control and the importance of dignity and the rights of each participant.
Aligns support to the individual needs, showing flexibility based on what is important to, and for, each person supported.
Uses principles of reasonable and necessary to support participant develop a quality tailored support plan.
Understands and navigates NDIS price guide and support categories to assist participants choose suitable supports.

Successful candidates must have a current “C" class driver’s licence and willingness to undergo a National Police Check prior to employment with DAHS.
