This position will assume a key role in the coordination and case management of National Disability Insurance Scheme (NDIS) services in the identified region.
The primary purpose of this position is to in accordance with the clients’ NDIS plans and needs manage the contract relationship and arrangements with the Client/Carer and review ongoing service needs, coordinate a team of care workers in the delivery of the agreed suite of services ensuring quality, timeliness, and effectiveness of service delivery. The role will be required to perform in accordance with Client Care plans, the Community Care Common Standards, and the SWAMS Policies and Procedures.
Specific requirements of this position include but are not limited to;
Essential Criteria
- Demonstrated knowledge of the NDIS and Disability services and the issues facing these organisations
- Knowledge of NDIS and experience working with people of all abilities
- Experience in delivering Support Coordination to NDIS participants or similar
- Demonstrated skill and/or ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander persons.
- An understanding of the health and social issues impacting upon Aboriginal people
- Well-developed understanding of care delivery in community care setting and within continuous improvement
- Excellent organisational and problem-solving skills
- Demonstrate respect and understanding of client issues including cultural appropriateness and anti-discrimination
- Demonstrate excellent oral and written communication skills
- Experience in undertaking complex assessment and care coordination of client services in a community, setting.
- Highly developed interpersonal skills including negotiation and consultation skills and the ability to proactively establish and sustain effective stakeholder relationships.
Desirable Criteria
- Aboriginal and Torres Strait Islander people are encouraged to apply
- Tertiary qualifications in Health and/Social Services sector.
