About the role

An opportunity exists in the Discharge Support & Liaison team in a permanent, part time position.

Applications are encouraged from Registered Nurses, Occupational Therapists, Physiotherapists, Social Workers, Speech Pathologists, Podiatrists and Dietitians with an interest in helping patient's access appropriate services to enable timely discharge from hospital.

The Discharge Support & Liaison team is responsible for working collaboratively within the multidisciplinary clinical teams to facilitate patient flow from the admitted inpatient setting through to the most appropriate pathway to prevent readmission and ensure that all patients receive the right care at the right time and in the right place.

Position Overview:

  • Permanent, part time (48 hours per fortnight)

What you bring

As a health professional with a minimum of 3 years' relevant experience, you will have developed knowledge and skills in the subacute and community setting. You will also have a demonstrated ability to participate in a changing organisation with creativity and flexibility, advanced skills in time management, organisation, planning, problem solving and prioritization. Highly developed interpersonal skills, an ability to work autonomously and collaboratively as part of a multidisciplinary team is also essential.

Employment Terms and Conditions will be in line with the Allied Health Professionals (Victorian Public Health Sector) Single Interest Enterprise Agreement 2021 - 2026 or Nurses and Midwives (Victorian Public Sector) (Single Interest Employees) Enterprise Agreement 2020-2024, with the appropriate discipline classification  Allied Health Professional Grade 2 / Registered Nurse Community Health Nurse.
