About the Role
We are looking for an experienced Care Manager or RN looking to take that next step to join the At Home Support team.
You will have acute care setting experience, leadership skills, supervision, delegation and sound critical thinking.
- Undertake admissions including complex ones as allocated.
- Use multiple approaches to clinical decision making.
- Coordinate the care of clients and their carer/s.
- Develop therapeutic and professionally appropriate relationships with clients and carers.
- Engage in collaborative practice to achieve client outcomes with peers and team members.
- Contribute to a supportive environment for colleagues.
- Act to enhance the professional development of self and others such as Graduates, Novices and Students.
- Provide services in accordance with funding guidelines for project initiatives.
Skills and Experience
- Bachelor of Nursing
- Current Senior First Aid Certificate and CPR
- Excellent communication and clinical documentation skills
- Experience working within Acute Ward in Hospital setting or similar environment.
- A commitment and high regard for safety and teamwork
- Influenza and Covid-19 Vaccinations
