St Charbel’s Care Centre is an organisation, dedicated to servicing the ageing population. SCCC is an integral part of the Maronite Community within Western Sydney and SCCC aims to allow consumers to retain their active role within their community.

We are currently seeking a Registered Nurse to join our team on permanent basis as the Documentation Nurse. Reporting directly to the Governance Manager, the Documentation Nurse is responsible for assessing consumer health, developing individualised care plans in consultation with consumers and their representatives, coordinating with the team, ensuring seamless service delivery, and maintaining compliance with health care standards.

Core responsibilities

• Assess new consumers on admission, working closely with the RNs and other multidisciplinary team members to ensure new admission needs are assessed, planned and implemented accordingly in consultation with consumers, families and other health professionals

• Formulate Care Plans on admission and periodically review/update care plans in accordance with changes in the consumer’s condition as required by the legislation

• Actively monitor and coordinate individualised assessments and care planning to optimise the health and wellbeing of consumers.

• Coordinate and actively participate in family and multi-disciplinary team conferences

• Liaise with medical practitioners and allied health care workers in consultation with other care staff for the provision of services to meet consumers’ needs

What You Can Offer Us

• Previous experience in similar role, can be from a different setting, e.g., community services, disability services and home care services.

• Current Registered Nurse registration with the Australian Health Practitioner Regulation Authority

• Experience with Aged Care IT systems

• Understanding of Aged Care Accreditation and Aged Care standards

• Critical thinking skills with excellent time management and attention to detail
