Form of work About The Organisation: This regional Aboriginal and Islander Community Controlled Health Service is an Aboriginal and Torres Strait Islander Community Organisation. Their objective is to provide increased access to quality primary health care, social and emotional wellbeing services to the Communities in the South Burnett regions. Key Duties:
Provide excellent clinical care, in keeping with accepted best practice standards, and the CRAICCHS model of care
Provide skilled health assessment, diagnosis and treatment services to patients
Perform and oversee the delivery of comprehensive preventive health assessments and prepare client care plans that comply with MBS standards, for clients across the age spectrum, and provide leadership for the multidisciplinary team in delivery of coordinated care planning for clients with complex chronic care needs
Order diagnostic tests as needed and checking and informing patients of results in accordance with the practice?s procedure
