About this role
The Hospital LiaisonGeneral Practitioner (HLGP) facilitates cross sector integration and coordination of care between Women and Newborn Health Service and community General Practitioners, in partnership with the WA Primary Health Alliance and local primary care services. The HLGP advises on and contributes to system improvements related to the general practice and hospital interface to optimise the continuum and quality of patient care.
This role reports to Director Clinical Services.
What we are looking for
We are looking for General Practitioner (Hospital Liaison) to join our team at Women and Newborn Health Service. You will be a positive, resilient individual actively working in active General Practice and/or can demonstrate extensive clinical experience in that setting. High level interpersonal and communication skills, both oral and written, and the ability to work with a diverge range of stakeholders is also essential to the role.
