The Role:
- Enhancement of the continuity and safety of patient care between the primary and hospital sector by identifying barriers to effective clinical handover and barriers to patient access to inpatient and outpatient services and leading activities to reduce these barriers.
- Monitoring and evaluation of communication between the primary care and hospital sectors.
- Receive and manage feedback from general practitioners regarding hospital services and patient advocacy.
The Team: * Based within the Clinical Management team at the Launceston General Hospital with support to the regional Hospitals
- Region to cover General Practitioners and Rural Medical Practitioners in the North of the state
- Reports to the Executive Director of Medical Services THS North
Details of Appointment
Fixed term, part time, day work, position working 20 hours per fortnight, commencing as soon as possible until 25th August 2023
*notwithstanding hours may be negotiated with the successful applicant

Visiting Medical Practitioner Level 1 - Visiting Medical Practitioner Level 2 $254,149 - $332,862 per annum, pro rata

Successful applicants will be required to meet the essential criteria:
General or limited registration with the Medical Board of Australia
*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee's responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered.
Applicants should note the following criteria are desirable: * Postgraduate qualifications in health services management or other relevant experience
