About the Role

The Health Coordinator will be a valued member of the Health Coordination Team and will be required to work in a role which provides support and coordination to clients who have recently been discharged from hospital. Our clients all have chronic and complex needs. We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity.

The Health Coordination Team comprises of experienced nursing and allied health staff. This team is very flexiable, knowedgable, friendly, and a highly skilled team of profesionals.  Clients are supported by the Health Coordinators in Health Education, Care Coordination and encouraged to be activley involved in making health decisions and making healthy lifestyle changes. The primary objective with Health Coordination is to acheive consistant care for clients between and across admitted and community based care, delivering care coordination that is short term to medium-long term.  Services are designed to reduce the lenght of hospital admissions, aid in increasing a clients self-managment and reduce readmission to hospital. 

Team members must be flexiable, reliable, passionate about helping others, have excellent communication skills, have knowedge and experience of supporting and educating clients in the area of self mangement and chronic disease management.   The sucessfull applicant must have a sence of humor, be hard working, and a team player.

Employment Type: This is permanent full-time position. However, applicants prefering a part time positon are also encouraged to apply.
