About the role
We are seeking a Health Care Worker(s) to join our enthusiastic team at Coleraine District Health Service.
The successful applicant will provide high standard services that support and assist residents to achieve maximum independence by meeting daily living and personal requirements within the plan of care, with consideration of individual preferences.
In conjunction with nursing staff you will contribute to the assessment and care planning, documentation and evaluation of care outcomes centred on evidence based practice.
The role will assist consumers to maintain their independence wherever possible and to provide daily care using a person centred approach in accordance to the resident’s individual care plans. The successful candidate will contribute to harmonious relationships within the team and work positively to achieve resident and team goals.
You will assist residents to maintain their independence wherever possible and to provide daily care using a person-centred approach in accordance to the resident’s individual care plans and also work with the Registered and Enrolled Nurse with resident care as directed, within the scope of practice.
Hours of work will include shift work, weekends and public holidays.
You will bring:
- Certificate III in Aged Care/ Individual Support
- Advanced communication skills including computer literacy
- Ability to work independently and as part of a team
- Person-centred approach
