Job description

BCR Communities (formerly Bay & Basin Community Resources) is an award winning outstanding community services organisation offering a range of support services across all ages and stages of life. We are a highly regarded local organisation which has been serving the Shoalhaven and Illawarra communities for over 30 years.

We are seeking a suitability qualified Registered Nurse or Allied Health professional (preferably OT) for the role of Home Care Clinical Lead to provide clinical oversight and operational guidance to ensure high quality, safe and meaningful client centred care across our aged care services.

Reporting to the Executive Manager of Aged Care Services, the Clinical Lead is responsible for overseeing the day-to-day clinical care and supports in our Home Care services, with a view to manage, monitor and review the program, ensuring compliance with Aged Care Quality Standards and the BCR Care and Clinical Governance Framework. The Clinical Lead will collaborate with the Executive Manager of Aged Care Services and other leaders across the organisation to review and update policies and procedures, develop and implement continuous improvement initiatives.

The successful applicant will work closely with the Clinical and Leadership teams and Care Managers to champion high quality and safe practice supporting continuous improvement. The successful applicant will also coach and mentor our team to deliver quality outcomes for clients and build a strong and positive culture. They will support with induction and professional development of staff and be responsible for high level supervision and leadership of the clinical team.  

Alongside these duties, the Clinical Lead will provide in-home clinical assessments and clinical support as required.

Location and hours of work are flexible across our offices in the Illawarra and Shoalhaven, including an option for partial work from home arrangement.  


About You:
• Registered Nurse or Allied Health professional with current AHPRA registration
• Previous leadership experience desirable
• Demonstrated knowledge and skills to provide safe and effective clinical care
• Excellent skills in assessing, documenting and implementing client care plan goals
• Experience in community care/home based care services
• Exceptional customer service skills
• Excellent communication and interpersonal skills.
• Highly developed organisational and prioritisation skills.
• Demonstrated self-awareness and ability to work autonomously.
• Proactive and focused on continuous improvement and experience in ensuring compliance to aged care standards and regulations (or similar)
