About the role:
We are seeking a dedicated Aged Care Worker to join our Burwood Home Care team, supporting Doncaster and surrounding suburbs. This position is ideal for individuals who have a passion for working with clients with complex care needs and who are committed to providing exceptional care and support. The successful candidate will have the opportunity to work closely with our more complex clients and provide direct care and domestic assistance duties, as well as social support and transport as required for clients.
In this role, You will:
- Provide high-quality care and support to clients with complex care needs, including but not limited to individuals with physical disabilities, chronic illnesses, dementia, and mental health challenges
- Contribute to implementation of personalised care plans and evaluation of clients goals
- Assist clients with activities of daily living, including bathing, dressing, grooming, medication management, and meal preparation, transfers, and domestic support
- Monitor and assess clients' health and well-being, reporting any changes or concerns to the appropriate parties
- Provide emotional support and companionship to clients, promoting social interaction and engagement
- Mentor and provide guidance to lesser qualified staff, including Home Care Workers and Domestic Support Workers
- Support the training of Home Care staff to enhance their skills and knowledge
- Collaborate with Case Managers and other healthcare professionals, including nurses, therapists, and physicians, to ensure coordinated and comprehensive care for clients
About you:

- Unqualified entry jobs available
- Certificate III/IV or equivalent in Individual Support (Aged or Disability) or equivalent (or currently studying a Diploma in Nursing or Bachelor of Nursing)
- Experience in a Home Care setting or providing domestics support is desirable
- Comprehensive knowledge in community care with a commitment to high standards of care for clients and their families
- Understanding of client directed care
