Home Package Coordinator - Aged Care

About the role:

This position is a key member of the Home Caring team,  providing assessments care planning, monitoring and referrals for all our clients. Your role is responsible for performing in home face to face assessments and partner with clients, their carers/families to assess their current situations and create care plans that enable maximum independence and a positive impact for our clients.

Key Responsibilities:

• Liaising with clients as well as their families on a regular basis, to assess client satisfaction and address concerns.
• Conduct comprehensive client assessments and development of care plans to support our clients at home.
• Collaborating with internal and external stakeholders to ensure seamless service delivery.
• Monitoring service delivery and address changes to client needs.
• Maintaining up to date client records management, including continual quality management and improvement.
• Managing HCP budgets to ensure client care needs & activities are met.
• Facilitate the transition of clients onto packages.
• Participate in objectives and performance review to reach KPI’s set in relation to new and existing Clients.

To successfully fill this role, you will have:

• Experience with Home Care Packages.
• Knowledge of Aged Care/Community Care quality standards.
• High level of verbal & written communication & influencing skills.
• Proactive problem-solving abilities & attention to detail.
• sensitivity to a culturally diverse workplace.
• Registered or Enrolled Nurse would be an advantage.
• A reliable vehicle and current unrestricted Driver’s licence.
• Demonstrated proficiency in Microsoft Office Suites and Computer based systems.
• Excellent verbal and written communication skills.
